Drinking and Your Blood Sugar
Alcohol can drop blood sugar hours after your last drink — the liver pauses sugar release to clear the alcohol first. Why lows hide, and who should plan ahead.
Here is the part that catches people off guard: the blood-sugar dip that comes with drinking often does not show up while you are drinking. It shows up later — the middle of the night after a few, or the next morning — long after the last glass. That delay is not random. It comes straight from how your liver handles two jobs at once, and understanding the trade-off it makes explains most of the blood-sugar surprises people notice around alcohol.
Why the liver drops the sugar job
Between meals and overnight, your liver keeps your blood sugar from sagging by releasing sugar it has stored. That is its steady background task. Alcohol interrupts it. The American Diabetes Association puts it plainly: the liver will prioritize breaking down alcohol over keeping blood sugar steady, and it tends to make that call when you drink without eating. The stored-sugar tap slows, and your level can drift lower than it otherwise would.
Now the timing makes sense. The liver stays busy clearing the alcohol for a while, so the sugar job stays on hold well past your last drink — which is why the low lands hours later instead of in the moment. NIDDK, the federal diabetes institute, lists "drinking too much alcohol without enough food" among the things that push blood sugar too low, noting that alcohol "makes it harder for your body to keep your blood glucose level steady, especially if you haven't eaten in a while."
There is a second, quieter problem, and it is the one that turns a nuisance into a risk. Alcohol can mute the early warning signs of a low. NIDDK notes that alcohol "can also keep you from feeling the first symptoms of low blood glucose." The trouble is that a real low and simple drunkenness feel a lot alike — shaky, sweaty, muddled — so the body's alarm gets read as the buzz and waved off. A low that gets mistaken for tipsiness is a low with room to get worse.
This is sharpest for anyone whose medication already lowers blood sugar. If you take insulin or a sulfonylurea — an older type of diabetes pill that pushes your body to make more of its own insulin — those pull glucose down on their own, and alcohol stacks on top. There, a delayed low is not a curiosity to note; it is the main thing to plan around.
Alcohol is one input, not the whole reading
A single glucose number rarely has one cause, which is exactly why it is hard to pin on the drink by itself. A sweet mixer, a skipped meal, a hard workout, a bad night's sleep, an illness, stress, and the timing of any medication can each move the same reading. Alcohol lives in that mix; it does not override it. NIAAA's overview of how alcohol travels through the body traces its reach across the brain, blood vessels, liver, pancreas, and digestive system — several of which touch glucose — which is part of why one drinking night can nudge a number through more than one door.
And it is a common question, not a niche one. In its 2024 summary, NIAAA reports that about 174.4 million U.S. adults — roughly 66.5 percent — drank in the past year. A large share of them also watch a glucose number somewhere, whether on a fingerstick, a yearly lab, or a continuous glucose monitor (the wearable sensor, often shortened to CGM, that samples your sugar every few minutes).
How much drinking, and how it lands
There is no amount of alcohol that is safe or harmful for everyone's blood sugar, because the effect depends on more than the pour. It scales with how much you drink and how often, and it lands differently from one person to the next. Whether you have eaten, which diabetes medications you take (if any), how well your liver is working, your body size, and how regularly you drink all shift where the line sits for you. Two drinks with a full dinner is a genuinely different event for your glucose than the same two on an empty stomach after a workout — same drinks, different outcome.
Two shared yardsticks help you describe a pattern instead of guessing at it. A U.S. standard drink is 0.6 fluid ounces, or 14 grams, of pure alcohol — one 12-ounce regular beer, one 5-ounce glass of wine, or one 1.5-ounce shot. NIAAA counts binge drinking as the pattern that brings blood alcohol to about 0.08 percent, usually 5 or more drinks for men or 4 or more for women in roughly two hours. Those are counting tools, not switches where harm turns on. But a heavier or emptier-stomach session is exactly the setup that makes a delayed low more likely — so an honest number is worth bringing to any conversation about your readings.
What actually lowers the odds
None of these is a cure, and none replaces a diabetes plan — if you manage diabetes or take glucose medication, your clinician's instructions come first. They are low-stakes habits that make the pattern easier to see and a delayed low less likely.
- Eat while you drink, not after. The empty-stomach version is where the liver is most likely to drop the sugar job, so having food in your stomach before and during drinking is the highest-value move here — especially at events where the drinks arrive well before the food does.
- Keep meals steady around a drinking night. Skipping dinner and running on a sweet mixer is a classic setup for feeling shaky hours later. Regular, balanced meals give your liver something to work with.
- Check afterward, and know the signs. If you monitor your sugar, a reading after drinking — and again before bed on a drinking night — catches the delayed dip the timing tends to hide. Take shakiness, sweating, a racing heart, hunger, and confusion seriously instead of writing them off as the drink.
- Leave gaps and keep some dry days. Spacing drinks out and keeping fully alcohol-free days gives you, and anyone helping you read your numbers, a cleaner before-and-after to compare.
- Bring the pattern, not one number. A short log — drinking days, meal timing, readings, symptoms, and your medication list — tells a clinician far more than a single startling value pulled out of context.

When a low is an emergency — and when it is something else
A severe low is not a wait-and-see. NIDDK notes that severely low blood glucose can cause someone to pass out or have a seizure, and can lead to coma or death. If you take diabetes medication, follow your own low-blood-sugar action plan for symptoms like shakiness, sweating, a fast heartbeat, or confusion — and call 911 if a low is severe, will not lift, or comes with loss of consciousness or a seizure.
One pattern gets mistaken for a glucose problem and is not. Shaking, a racing heart, repeated vomiting, agitation, confusion, hallucinations, or a seizure that appear after cutting down or stopping alcohol can be signs of alcohol withdrawal, which can be dangerous. That is a call-911-or-go-to-the-emergency-room situation, not something to fix with a snack or a glucose experiment.
A cutback, for its part, is not a promise that a lab value will move. Drinking less can make your patterns easier to read, but it will not on its own normalize an A1c (the lab number that reflects your average blood sugar over about three months), a fasting glucose, or your time-in-range on a monitor — and it is never a reason to change, skip, or re-time a diabetes medication on your own. Those adjustments belong with the clinician who manages your care, along with anything that touches a diagnosis: pre-diabetes, hypoglycemia that keeps happening, or a lab result you cannot explain.
