Alcohol-related liver damage runs a spectrum: fat, then inflammation, then scarring, rising with how much and how long. What a flagged AST can and cannot tell you.
A U.S. standard drink contains 0.6 fluid ounces, or 14 grams, of pure alcohol (CDC). That single figure is where most liver math should start, because nearly every other number on this page — a lab value, a guideline limit, a mortality count — only means something once a "glass" has been converted into standard drinks.
Start with the reference point the liver works against. The liver breaks down most of the alcohol a person drinks, and that processing is not a clean transfer. It produces byproducts, changes the normal chemistry inside liver cells, and over time can drive fat buildup, inflammation, and scarring. AASLD describes alcohol-associated liver disease as a spectrum running through three conditions in particular: hepatic steatosis (fatty liver), alcoholic hepatitis (an inflamed liver), and cirrhosis (permanent scarring). Those are not three separate diagnoses so much as three points along one gradient — fat inside liver cells, then inflammation, then scarring — and the count that matters is how far along that gradient a given liver has moved.
The mortality figures, and what they count
Liver disease shows up at population scale before it resolves into any single number. NIAAA's account of alcohol and the human body puts a number on the share: of an estimated 96,610 liver disease deaths among people ages 12 and older in 2023, about 43,004 — roughly 44.5 percent — involved alcohol. NIAAA Surveillance Report #123 tracks that cirrhosis mortality through 2023. Even within liver disease, alcohol accounts for close to half the deaths but not all of them.
The denominator behind those counts is looser than it looks. In Surveillance Report #123, "alcohol-related" rests on alcohol being mentioned in a death record — not a full reconstruction of everything that contributed to one death. As a signal across hundreds of thousands of records, the share is informative. Applied to one person, it is a blunt instrument: a population rate is not a personal probability.
The lab numbers: injury markers versus function markers
A flagged enzyme is a single data point, and its meaning depends on which number it is. AASLD separates liver injury markers from liver function markers: ALT, AST, ALP, and GGT flag patterns of injury, while albumin, PT/INR, and bilirubin measure how well the liver is doing its actual jobs. The distinction matters because the two sets answer different questions — one asks whether cells are being damaged, the other asks whether the organ is still functioning.
Two AASLD specifics constrain how far a single value can be read. First, ALT is more specific to liver injury, while AST can come from other tissues, including muscle and heart — so an AST elevation can reflect a hard workout rather than the liver. Second, the size of an AST or ALT elevation does not cleanly track how much liver injury exists; a larger number is not a proportionally larger diagnosis. AASLD notes that an AST/ALT ratio above 2 is common in alcohol-associated liver disease — a pattern, not a verdict. The full panel is wider than the one line that gets flagged. The drink count, in standard units
Self-reported drinking is the input most often misstated, because the unit drifts. A "glass" is not always a standard drink: a strong pour or an oversized wine glass can exceed one of the 0.6-ounce units defined above — and 0.6 fluid ounces is the size of one drink, not a daily limit. Converting an evening's drinks into standard drinks, and counting drinking days per week, is what makes every other number comparable.
What the evidence does say plainly is that liver risk is dose-related: alcohol-associated liver disease usually follows years of heavier drinking, and the chances of it rise the longer a person drinks and the more they consume. The same amount lands differently from one person to the next: AASLD ties how the disease progresses not only to the amount but to female sex, genetic susceptibility, diet, and other liver conditions.
The published reference point comes in standard-drink terms. The 2020-2025 Dietary Guidelines for Americans say adults who drink should limit intake to 2 drinks or less in a day for men and 1 drink or less for women. That is a yardstick for whether a usual pattern sits above a public-health limit — not a liver-safety threshold and not an all-clear. The number's job is to describe a pattern accurately to a clinician.
What the numbers don't settle
The figures above are firmest at scale and weakest as personal prediction. The same standard-drink pattern does not produce the same liver in two people: genetics, nutrition, viral hepatitis, metabolic health, medications, age, and the exact shape of someone's drinking all move the outcome, and none of those variables appears in a cirrhosis mortality share or an AST value.
The lab numbers carry the same ceiling. An enzyme level marks a question, not a quantity of disease; a mortality code marks an association in a record, not a cause established for one person; a guideline limit marks a population reference, not an individual risk line. The most a symptom or a single lab line supports is "worth a clinical conversation." Cutting back changes the direction, and how much depends on the stage. A shorter list is urgent now — vomiting blood, severe confusion, severe abdominal pain, fever with jaundice, or rapidly worsening swelling.
One factor the data does not measure is stigma. AASLD recommends the term "alcohol-associated" over "alcoholic" specifically to reduce that stigma. That framing is not a diagnostic input: the liver's state is established by the panel and the clinical picture, not by how the reader feels about the cause.
